The Adult Day Care Center provides participants with a wide range of activities which provide an opportunity to interact socially with peers, share in stimulating activities, and receive assistance with activities of daily living. Sometimes referred to as Senior Day Care, the Adult Day Program serves both seniors and adults with disabilities. In addition to group activities, each participant has an individualized plan of care for Adult Day Services to address their unique needs and goals.

 

 Overview of Adult Day Services:

 

  • Individual and group activities
  • Assistance with activities of daily living
  • Engagement in sensory stimulation
  • Nutritious meals
  • Exercise and recreational activities
  • Learning opportunities that can include reading, computer skills development, subject discussions, local and national news, family and group discussions, spiritual needs, life lessons
  • Relaxation activities that can include movie/tv viewing, music, games, cards, bingo, humor therapy, story time
  • Medication assistance if indicated in service plan
  • Coordination of physical therapy if indicated in service plan
  • Coordination and/or provision of other services if indicated in service plan

Personal Support Services are typically provided in the homes of clients or their caregivers. The services are individualized and based on the needs of the client and family. The services can include assisting the client with activities of daily living such as bathing, grooming, toileting, and feeding. Household assistance can be provided to help the client and family maintain a safe, sanitary, and nurturing environment.

Respite for caregivers can be provided for short periods to relieve the primary caregivers of their duties of caring for their loved ones.

 

Overview of Personal Support Services:

 

  • Provides assistance with activities of daily living
  • Assists patients with meal planning and preparation
  • Cleans rooms and makes patient beds
  • Converses with patients to provide assurance
  • Provides patient laundry and linen changes
  • Light housekeeping
  • Provides medication reminders
  • Planning and preparing meals according to specific dietary requirements of the client
  • Maintaining of a safe, clean and healthy environment through light housekeeping including changing bed linens, dusting and vacuuming, cleaning kitchen and bathroom, and laundry
  • Provide companionship and stimulation for the client including reading, walks, etc.; and accompanying the client to doctors or other appointments
  • Performing other housekeeping tasks as indicated in the care plan

 

The specific Personal Support Services provided for each client are based on the individualized plan of care.

Community Living Support is a community based residential alternative service for senior care and adults with disabilities encompassing a continuum of support option for up to four individuals living in a home that supports each resident’s independence and full integration into the community, ensures each resident’s choice and rights, and complies fully with standards applicable to Home and Community Based Settings.

CLS services are individualized based on the needs of each resident and specified in the person-centered plan of care, but may include hands-on assistance, supervision, transportation and other supports needed to help the individual:

 

  • Select and move into a home
  • Locate and choose suitable house mates
  • Acquire and maintain household furnishings
  • Acquire, retain or improve skills needed for activities of daily living or assistance with activities of daily living as needed, such as bathing, dressing, personal hygiene and grooming, eating, toileting, transfer and mobility
  • Acquire, retain or improve skills needed for instrumental activities of daily living or assistance with instrumental activities of daily living as needed, such as household chores, meal planning, shopping, and managing personal finances
  • Build and maintain interpersonal relationships with family and friends
  • Pursue educational goals and employment opportunities
  • Participate fully in community life, including faith-based, social and leisure activities selected by the individual
  • Schedule and attend appropriate medical services
  • Self-administer medications, including assistance with administration of medications as permitted pursuant to TCA 68-1-904 and TCA 71-5-1414
  • Manage acute or chronic health conditions, including nurse oversight and monitoring, and skilled nursing services as needed for routine, ongoing health care tasks, such as blood sugar monitoring and management, oral suctioning, tube feeding, bowel care, etc.
  • Become aware of and effectively use transportation, police, fire, and emergency help available in the community to the general public
  • Assert civil and statutory rights through self-advocacy

About Us

LifeCare Foundations is a faith based non-profit organization founded to provide services to seniors and those with disabilities to ensure a high quality of life is maintained in the community based setting.

 

Licensed by the TN Department of Human Services and the TN Department of Mental Health and Substance Abuse Services, LifeCare Foundations opened mid-year 2010 initially to provide Personal Support Services. The Adult Day Program opened in late 2010. LifeCare Foundations grew out of the need recognized by its founder, LifeCare Family Services, to serve the senior population and those with disabilities. LifeCare Family Services is a licensed community mental health center that offers a full range of behavioral health services to meet the needs of children, adults, and families. In 2010, LifeCare Family Services expanded its array of services to include primary care.

 

Integrated care is at the forefront in today’s health care environment. LifeCare Foundations and LifeCare Family Services offer a unique form of integrated care in which the client can receive long term care, behavioral health, and primary care within the same facility. The LifeCare Foundations Adult Day Program has the added benefit of having access to daily nursing services provided by the LifeCare Foundations nursing staff and the LifeCare Family Services nursing staff.

The mission of LifeCare Foundations is to provide services to seniors and those with disabilities to ensure a high quality of life is maintained in the community based setting. Maintaining the family unit during all phases of the life span is a priority. The agency is committed to providing services in the community to assist and support families to continue to care for their loved ones at home and avoid premature or unnecessary institutionalization.

Philosophy

LifeCare Foundations operates under the philosophy of “person centered care.” Person centered care embodies the belief that the individual should be treated with dignity, respect and love. Care should be individualized based on the specific needs of the client and family. Helping people and their loved ones maintain health and independence for as long as possible is a primary goal. Professional, well trained, committed staff members are vital to this philosophy.
