The Opportunity

The Direct Support Professional will provide care to seniors and adults with disabilities in an innovative, community-based home environment.  Direct Support Professionals (DSPs) play a critical role in providing assistance and support to individuals with intellectual and developmental disabilities, helping them to lead fulfilling lives and participate more fully in their communities. These responsibilities may include assisting with daily activities such as personal care, meal preparation, medication management, and transportation, while also offering emotional support and encouragement. DSPs will work closely with individuals, their families, and other healthcare professionals to develop and implement personalized care plans that promote independence, dignity, and overall well-being.

Job Duties & Requirements

  • Provide support and assistance to residents in a variety of areas (domestic, personal hygiene, work, social and recreational skills, self-help skills)
  • Provide coordination of outpatient care for residents in a variety of areas (primary care, specialty care, dental, hearing, vision)
  • Observe and report changes to residents’ health and behaviors
  • Arrange and actively participate in residents’ activities including holidays, day trips, and seasonal events
  • Prompt and assist residents with activities of daily living
  • Assist with meal planning and preparation
  • Provide services to residents based on the Plan of Care
  • Ensure all services are provided in accordance with licensure and BHO standards
  • Ensure home and grounds are clean and orderly at all times
  • Thoroughly document progress notes
  • Collaborate with all stakeholders to develop Plans of Care
  • Report and review all resident medical concerns with the nursing staff
  • Collaborate with Housing Manager on quality of care issues
  • Report and review all incident reports with the Housing Manager and Director
  • Maintain compliance with all agency policies
  • Other duties as appropriate and as needed that are essential to meet licensure and MCO requirements
  • Medical Assistant, CNA or CNT preferred, but applicants with significant caregiving experience for seniors or adults with disabilities will be considered
  • High School Diploma or GED
  • Valid Tennessee Driver License
  • Must pass Nationwide Criminal Background Check
  • Experience with TennCare Choices Program preferred but not required
